Field RN Assessor: HealthCare Support seeks a Field RN Assessor to fill a Fortune 100 company opening.

Daily Responsibilities For Field RN Assessor

• Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development
• Monitor delivery of services and follow-up with members, caregivers, or providers through in-person visits and telephonic contact
• Authorize and coordinate referrals for services
• Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care
• Assist in coordinating the development of informal or voluntary services to integrate into the member care plan
• Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services
  
  
  

Required Qualifications For Field RN Assessor

• Bilingual in English and Spanish, HIGHLY preferred
• NY State RN License
• 2+ Field Case Management/Home Health or LTC experience
• New York State Driver’s License and access to vehicle

• can be a family member who drives them around or a rental car service but will only be reimbursed for mileage
  
  
  

Preferred but not a MUST:

• BSN preferred
• Home Health/LTC or Nursing home experience
• come from a licensed home care facility
• Bilingual in Spanish – huge plus
• Previous Acute care background from a hospital setting

Pay Details: $56 to $60- per hour.

Location: In New York, those who are willing to drive to the below locations preferred locations – may need to go to the office quarterly

• Bronx (Highly preferred) – Yonkers or the southern part of West Chester
