Job summary

Community Healthcare of Texas is recruiting a Field RN to meet with patients and families, explain the scope of hospice services, coordinate admission of patients, and develop ongoing plans of care.

The ideal candidate holds a Texas RN license and a valid driver’s license, with 2+ years of nursing experience (hospice experience preferred). Computer proficiency and accurate charting are required to support patient care and collaboration with the Case Manager.

Qualifications

• Registered nurse licensed in Texas, with current license and credentialing.
• Two years of nursing experience required, hospice experience preferred.
• Proficient computer skills and ability to document accurately.

Responsibilities

• Meet with patients and families to explain hospice services and plan of care.
• Coordinate admission of patients and ongoing care plans; update charts and documentation.
• Assess patient condition and revise the plan of care as needed; communicate with Case Manager.
• Provide emotional support and guidance to patients and families.

Community Healthcare of Texas is currently recruiting for a Field RN.

POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

ESSENTIAL FUNCTIONS

• Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
• Provide emotional support to patients and families while instructing them in activities of daily living.
• Complete daily paperwork and charting, thoroughly and accurately, according to Policy.
• Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.

POSITION QUALIFICATIONS

• Registered Nurse in the State of Texas
• Hold a valid driver's license and have a dependable means of transportation.
• 2 years of nursing experience required
• 2+ years of experience in hospice strongly preferred
• Proficient skill using a computer is required; Microsoft Office Suite experience preferred.
