What to Expect (Job Responsibilities):
- Conduct telehealth and in-home assessments on patients in designated areas, including medication review and health screenings
- Provide patient education to align care desires with the best available options
- Identify diagnoses for care management and active medical management
- Collaborate with the Regional Medical Officer and team members to develop appropriate care plans
- Coordinate care with various stakeholders, including PCPs, specialists, and ancillary providers

What is Required (Qualifications):
- Master's degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
- Active California state Nurse Practitioner or Physician Assistant license
- Active RN license and furnishing number (for NP only)
- Active Nurse Practitioner Board Certification or Physician Assistant Certification
- Valid California driver’s license and current automobile insurance

How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous EMR experience
- Experience in the care of older adult (geriatric) patients
- Excellent administrative, organizational, and communication skills
