Overview of the Role:

The Alignment Care Anywhere program provides medical and social support to our patients in their homes, aiming to prevent hospitalizations, health complications, and unmanaged disease progression through timely clinical interventions.

Responsibilities:

  • Conduct in-home assessments, medication reviews, and health screenings.
  • Provide patient education tailored to care needs.
  • Identify diagnoses for care management and active medical management.

Partner and communicate with Regional Medical Officers and team members to develop appropriate care plans based on patient needs.

Coordinate care with PCPs, specialists, and other providers.

Lead a clinical team that may include nurses, health coaches, social workers, and care coordinators.

Requirements:

  • Master’s degree from an accredited NP or PA program.
  • Preferred one year of clinical or home care experience.
  • Previous EMR experience preferred.
  • Experience with geriatric patients is a plus.
  • Hospice/palliative care experience is preferred.
  • Active state Nurse Practitioner or Physician Assistant license.
  • Active RN License and Furnishing number (NP only).
  • Board Certification in Nurse Practitioner or Physician Assistant.
  • Valid driver license and current automobile insurance.

Physical Demands:

The physical requirements include talking, hearing, standing, walking, sitting, using hands, reaching, and lifting up to 20 pounds. Specific vision abilities include close vision and focus adjustment.
