The Advanced Practice Clinician in the Care Anywhere (CAW) program is designed to provide care and support to our patients in the comfort of their own homes. This program is offered to eligible patients at no cost, aiming to prevent unnecessary hospitalizations, health complications, and unmanaged disease progression through timely clinical interventions.

Responsibilities:

1. Conduct in-home assessments, medication reviews, and health screenings
2. Provide patient education tailored to care goals
3. Identify diagnoses for care management and active medical management
4. Collaborate with Regional Medical Officer and team to develop care plans
5. Coordinate care with stakeholders including PCPs, specialists, and ancillary providers
6. Lead broader clinical team including nurses, social workers, and care coordinators

Job Requirements:

Experience:

• Preferred: 1+ year of clinical or home care experience; EMR experience preferred; geriatric care experience preferred

Education:

• Required: Master's degree from an accredited NP or PA program

Specialized Skills:

• Effective communication skills
• Problem-solving, organizational, and time management skills

Licensure:

• Active Nurse Practitioner or Physician Assistant license
• Board certification, NPI, DEA, BLS, valid driver's license, and insurance

Physical Demands:

Regular talking, hearing, standing, walking, sitting, using hands, reaching, and lifting up to 20 pounds; specific vision abilities required.

Pay Range:

$130,332 - $195,498, based on factors such as location, education, experience, and responsibilities.
