Alignment Health in the United States is seeking a Field Remote Nurse Practitioner or Physician Assistant to support the Care Anywhere program. You will perform in-home assessments for members two days a week and work from home three days, coordinating care with PCPs, specialists, and the clinical team.

The role requires an NP or PA with a master's degree and California licensure, EMR experience preferred, and strong communication, problem-solving, and time-management skills.

Competenze

• Master's degree from an NP or PA program.
• Active NP or PA license for CA required.
• Board certification for NP or PA.
• NPI, DEA, and BLS credentials.
• Driver license and insurance.

Mansioni

• Conduct two in-home assessments per week for Alignment members.
• Review medications, perform health screenings, and provide education.
• Identify diagnoses for care management and active medical management.
• Coordinate care with PCPs, specialists, and other providers.
• Lead and collaborate with clinical teams including nurses and care coordinators.
• Communicate with Regional Medical Officer to develop care plans.
