Texas Children's Hospital is seeking a Field Relations Coordinator to grow with our team. You will deploy team-based population management and foster positive relationships with physicians, families and providers while coordinating screenings and care plan interventions where licensure is not required.

• Strong communication and collaboration with care coordination teams.
• Customer-focused service delivery and education to members and providers.

Responsibilities

• Develop and maintain satisfactory business relationships with participating providers.
• Collaborate with Service Coordinator, Care/Case Manager, and other team members to monitor care plan outcomes.
• May lead as the service coordinator for STAR Kids Level of Care 3 members with support from a Care/Case Manager.
• Review information with Member/family and develop care plans and self-management plans.
• Identify options and coordinate linkage to specialists, homecare, financial assistance, housing, meals, transportation, and community resources.
• May travel to member homes for periodic screening of unmet needs and education.
• Monitor service delivery to members including satisfaction and quality of services.
• Deliver exceptional customer service to contracted providers and Members.
• Ensure thorough communication and education to Members/providers about delivery system capabilities.
