We are looking for a **Per Diem** Registered Nurse (RN) for our Massachusetts team.*This is a hybrid position that will include **fieldwork** in the **Springfield****area** and working from home as well as an occasional commute to our **Boston, MA** **FreedomCare** **Office**. Please note that this is a per diem role.***Department & Position Overview:** Adult Foster Care is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient. In this position, as a Registered Nurse, you will perform clinical evaluations in the home of the Adult Foster Care member to assess the needs of the patient to ensure appropriate measures are provided for enrollment, development of service plan and ongoing compliance. You will also conduct monthly visits with members who are On Care with FreedomCare to ensure they are receiving quality service and address any changes in their condition. **Every Day You Will:*** Provides support to the caregiver and member, and reviews services according to scope of Adult Foster Care.* Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.* Follows the FreedomCare’s clinical policy and procedures.* Assesses and regularly reassess the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.* Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.* Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.* Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.* Interprets the philosophy and objectives of the agency to clients, families, and others.* Performs other job-related duties as may be required or directed by the supervisor.**Ideal Candidate Will Possess:*** Registered Nurse licensed by Massachusetts Board of Registration in Nursing* Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities* Bilingual in Spanish is a plus* Experience with CRM (Salesforce is a plus)* Valid driver’s license with an insured vehicle for travel within the state* Strong communication and interpersonal skills.* Problem-solving and decision-making abilities.* Organizational and multitasking skills.* Excellent communication, critical thinking, and organizational skills* Self-motivated* Able to thrive in a fast-paced start-up environmentWe are here for you and committed to providing a best-in-class employee experience. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this per diem role is between $46.00 and $53.00 *per hour* at the commencement of your first year of employment.
