The position involves conducting in-person assessments across Manhattan, close to public transportation, shopping centers, and vibrant neighborhoods.

Duties and Responsibilities:
- Conduct home and virtual assessments for prospective and current members using the Uniform Assessment System (UAS); determine enrollment and eligibility through clinical expertise.
- Assess service needs based on CMT and family/caregiver requests to determine necessary adjustments.
- Review previous assessments and medical notes to update care plans or facilitate nursing home admissions.
- Perform in-person care management visits supporting person-centered planning.
- Liaise between members and CMT; evaluate home environment and psychosocial factors.
- Provide feedback to home care agencies and CMT regarding care issues and staff performance.
- Offer community-based training on self-care and prevention strategies for frail populations.
- Ensure members and families understand and agree with enrollment in managed long-term care plans.
- Additional duties as assigned.
Minimum Qualifications:
- New York State RN license
- Ability to travel within downstate NY (5 boroughs, Long Island, Rockland, Westchester)
- For PEDS: 1 year of pediatric clinical experience or experience with families and child-serving systems, including child welfare or medically fragile populations.
Preferred Qualifications:
- Experience with home assessments, planning, and evaluations in community health or home care settings.
- Experience with electronic health records, especially UAS.
- NY driver’s license.
- Knowledge of Medicaid, Medicare, and MLTCP.
- Experience working with frail adults or elderly populations to develop care plans.
- Strong time management, communication, and problem-solving skills.
- Language skills in Spanish, Russian, French, Creole, Mandarin, or Cantonese.
- Ability to handle heavy caseloads.
- Proficiency in Microsoft Word, Excel, Outlook.
Compensation Range:
- Greater NYC Area (NY, NJ, CT): $100,000 - $120,000
- Other locations within approved areas: $71,600 - $106,505
Salary depends on experience, education, licenses, and other factors. Benefits include health coverage, life insurance, 401k, and more. The hiring range reflects good faith estimates for new hires or transfers.
