Job summary

Nursing & Therapy Services Of Colorado is seeking a full-time Occupational Therapist to provide direct patient care in home health and outpatient settings in Colorado Springs/Pueblo. You will evaluate functional needs, develop and implement plans of care, and collaborate with families and the care team to achieve optimal outcomes.

Minimum education requires graduation from an OT program accredited by AOTA with current Colorado licensure or eligibility.

Qualifications

• Graduation from an occupational therapy program accredited by the American Occupational Therapy Association.
• Master’s degree in related field preferred.

Responsibilities

• Provides occupational therapy in home health or outpatient settings.
• Evaluates functional needs and outcomes, and adjusts care plan accordingly.
• Communicates effectively with clients, families, and care team to achieve optimal outcomes.
• Maintains records of prescriptions, evaluations, treatments, and goals as required.

Designs and implements a plan of care based on evaluation of patient status.Conducts provision of therapy within accepted standards of care and in accordance with state law.Provides occupational therapy in a home care setting, or outpatient clinic as assigned.Communicates effectively with clients, relatives, professional and support staff to achieve desired patient outcome.Adheres to established organizational policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.Maintains current and accurate records of properly signed prescriptions, patient evaluations, treatments, goals and plan of care.
