Healthcare Support Staffing, Inc. is seeking a Field Nurse Practitioner or Physician Assistant to provide in-home, patient-centered care across Southeast Phoenix and East Valley, AZ.

You will serve as the primary care provider for Medicare/Medicaid patients with autonomy and a flexible approach to care. The role features a Monday–Friday 8am–5pm schedule with no weekend work, offering a salary of $120,000–$142,000, up to 7.5% annual bonus, mileage reimbursement, and a full benefits package

Qualifications

• Active Arizona NP or PA license.
• 1+ year of primary care, family practice, or outpatient experience.
• Active DEA license.
• Current BLS certification.

Responsibilities

• Conduct 5 to 7 daily in-home visits, assess patients, diagnose, prescribe treatments, and manage ongoing care.
• Educate patients on chronic disease management, coordinate with care teams, and document in Athena EMR.

Pay Details

• $120,000 - $142,000 annually (DOE)
• Eligible for up to 7.5% Annual Bonus
• Mileage Reimbursement (IRS rate for travel over 30 miles)

Shift

• Monday – Friday | 8am-5pm
• No weekends

Daily Responsibilities for the Field Nurse Practitioner / Physician Assistant

• Conduct 5 to 7 daily in-home and occasional telehealth visits, performing comprehensive patient assessments, diagnosing conditions, prescribing treatments, and managing ongoing care plans
• Educate patients on chronic disease management, coordinate care with interdisciplinary teams, and maintain accurate documentation in Athena EMR
