As a Field Nurse, you will support a panel of patients through routine and urgent home visits, combined with panel support and care coordination activities. These include making outbound calls to facilitate care plan implementation and coordinating with external care providers to ensure continuity of care.

Key Details
  • Location: Primarily field-based (75%+), with capability for virtual care activities from home.
  • Reports to: Clinical Nurse Manager
  • Daily work: Conduct home visits under clinical supervision.
  • Collaboration: Work with providers, RNs, nurse case managers, social workers, pharmacists, community health workers, and care navigators.
Key Responsibilities
  • Provide nursing care through routine and urgent in-home visits under the supervision of an authorized health care practitioner.
  • Facilitate virtual visits to support care delivery.
  • Perform clinical procedures such as blood draws, specimen collection, medication administration, wound care, and suture removal, all under supervision and with competency evaluation.
  • Coordinate and support patient care by implementing care plans and performing outbound calls and tasks.
  • Engage patients and establish trusting relationships, providing tailored care.
  • Deliver exceptional customer service through proactive and comprehensive communication.
  • Maintain adherence to clinical guidelines, policies, and best practices.
  • Participate in team meetings and adhere to healthcare laws and standards.
  • Other duties as assigned.
Job Specifications
  • Education and License: Active, unrestricted LPN or LVN license in the practicing state.
  • Home Visit Requirements: Ability to conduct independent home visits, willingness to travel 75%+ within the area, and proof of vehicle insurance.
  • Driver Requirements: Licensed for at least 5 years, with no recent suspensions or violations such as DUI or reckless driving.
  • Experience and Certifications: IV or blood draw certification, current BLS certification, 1+ years nursing experience, proficiency in clinical skills.
  • Preferred: Experience in urgent/community/home settings, complex care, geriatrics, or palliative medicine.
  • Knowledge and Skills: Caring for adults with complex needs, computer proficiency, familiarity with electronic health records and patient portals.
  • Personal Traits: Adaptability, teamwork, commitment to quality care, and serving underserved populations.
Vaccination Policy

All frontline workers must be fully vaccinated, with documentation provided. Exemptions for medical or religious reasons will be considered.
