Job summary

Carlisle Medical, Inc. seeks a Field Nurse Case Manager to cover Nashville, TN and surrounding areas.

The role requires RN licensure and CCM certification if applicable, with a focus on coordinating medical care for injured workers and ensuring efficient, cost-effective treatment. Responsibilities include assessing and planning case management, liaising with insurers, attorneys, and healthcare providers, arranging appointments, and maintaining complete case documentation for regulatory

Qualifications

• Must be an RN in good standing with CCM certification if applicable.
• Two years of direct clinical care experience recommended.
• Valid driver's license required.

Responsibilities

• Assess, plan, coordinate, implement and evaluate injured workers throughout the case management process.
• Serve as liaison among insurers, attorneys, providers, employers and employees to facilitate return to work.
• Coordinate appointments and participate in treatment planning to ensure cost-effective care.
• Maintain detailed case documentation and prepare progress and billing reports.
• Keep current on workers’ compensation laws and participate in professional associations.
