SYNERGY Home Care Franchising, LLC is seeking Registered Nurses/Case Managers to join our Clifton-Secaucus operation as part of our national agency. You will assess clients, plan care, and collaborate with families and care teams to deliver quality in-home services across multiple New Jersey areas.

As an RN/CM, you will establish professional relationships with clients and caregivers, provide reliable in-home care, and support ongoing training for staff.

Qualifications

• RN license required and current in the state of New Jersey.
• Experience in home care or case management preferred.
• Ability to develop and adjust care plans with input from families and clinicians.

Responsibilities

• Assist with employee training as needed, utilizing your expertise as a registered nurse.
