Job summary

Universal Healthcare MSO LLC seeks an experienced LVN Case Manager to deliver Enhanced Care Management (ECM) services in Fresno, CA. The role focuses on addressing complex medical and social needs through coordinated, person-centered care, primarily via in-person interactions at members’ residences or communities.

The position is full-time, non-exempt and hourly paid, with a work schedule Mon–Fri 8:00 a.m.–5:00 p.m. PT. Competitive compensation and benefits are offered.

Qualifications

• High School diploma or GED required.
• LVN license active in California.
• Minimum 2 years in a healthcare setting.
• Experience with case management and care coordination.
• Ability to communicate effectively with diverse teams.

Responsibilities

• Effectively manage and maintain a caseload of ECM members.
• Conduct a comprehensive assessment to develop an individualized care plan.
• Engage with members in-person to provide culturally appropriate communication.
• Identify resources to address health and social needs.
• Ensure Care Management Plan is updated under clinical oversight.
• Coordinate interdisciplinary meetings and referrals.
