Job summary

Community Healthcare of Texas is seeking a Field Licensed Vocational Nurse in Fort Worth, TX, to support the RN Case Manager with the plan of care and patient care coordination. This full-time field position focuses on delivering compassionate hospice services in the community.

The LVN will document care, administer medications, and participate in on-call rotations, with a sign-on bonus offered. Prior hospice or oncology experience is preferred, and a valid Texas LVN license is required.

Qualifications

• Active LVN license for the State of Texas is required.
• 1 year of nursing, hospice, or oncology experience preferred.
• Must hold a valid driver's license and have reliable transportation.
• Computer skills including Microsoft Office are preferred.

Responsibilities

• Assist the RN Case Manager in the plan of care for the patient.
• Coordinate with team members and communicate with IDG per policies.
• Document clinical notes and changes in condition.
• Administer medications and treatments as prescribed by the physician.
• Record nursing care and interactions with patients and staff.
• Assess patient/family response to care and participate in follow-up with bereaved families.
• Participate in on-call rotation.

POSITION SUMMARY

Assist the RN Case Manager in the plan of care for the patient.

ESSENTIAL FUNCTIONS

• Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures, and regulations that define the scope of practice.
• Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate.
• Administer medications and treatments as prescribed by the physician.
• Document nursing care and significant interactions with patients, support persons, and team members.
• Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact.
• Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
• Participate in on-call rotation

• Licensed Vocational Nurse licensed by the State of Texas, required
• Hold a valid driver’s license and have a dependable means of transportation
• 1 year of nursing, hospice, or oncology experience preferred
• 1 year LVN experience in an acute care setting required.
