Job summary

Evolve Home Health is seeking a Licensed Vocational Nurse (LVN) with home health experience to cover cases throughout California, including Coalinga and surrounding areas. The role requires travel up to 100% of the time and provides per-visit compensation.

Patients are cared for in home settings with focus on wound care, dementia care, tracheostomy care, and adherence to Medicare/Medicaid hospice guidelines. Valid CA LVN license and BLS/CPR/ACLS are required.

Qualifications

• Must hold a California LVN license.
• 2–3 years LVN experience in home health/hospice.
• Proficient with electronic health record systems (DataSoftLogic).
• Familiar with DME in patient care.

Responsibilities

• Provide nursing and supportive care per the Plan of Care under RN supervision.
• Communicate with physicians and care team to coordinate care.
• Document all services on required forms.
• Administer medications and perform wound/trache care per orders.
• Monitor patient condition and report changes to RN.

Job description

We are looking for Licensed Vocational Nurse (LVN) with a background in home health care experience. Applicants must have prior experience in home health nursing to be considered for this role.

Our agency Evolve Home Health is located in Agoura Hills, CA, we provide services throughout Los Angeles County and extending into the Mid/Central Valley. (Fresno, Clovis, Madera, Coalinga, Mendota, Merced, Tulare, Visalia, San Diego area, and Los Angeles County)

We currently have open cases in COALINGA, TULARE, AND VISALIA.

ESSENTIAL FUNCTIONS/SKILLS

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
• Great time management skills
• Performs wound care and Wound Vac
• Dementia care
• Tracheostomy care

VISIT TYPES

• Routine/Follow-Up visits: monitoring chronic conditions
  
• Take vital signs, weight, and basic assessments
• Wound care under standing orders
• Skilled nursing visits under RN Delegation: Administering medications; Trache care; ostomy care
• Dressing changes, wound care maintenance
• Post-Discharge or Resumption of Care Visits
• Checking on patient progress and reporting back to the RN or DCPS
• Patient Education/Support

QUALIFICATIONS

• Current Licensed Vocational Nurse (LVN) license in the state of California
• Proficiency in using electronic health record systems (e.g., DataSoftLogic)
• Familiarity with durable medical equipment (DME) and its use in patient care
• Minimum of 2-3 years as a Licensed Vocational Nurse (LVN)
• Valid Drivers License, reliable mode of transportation, and proof of insurance
• BLS/CPR/ACLS Card
  
• Previous Hospice experience required

OTHER REQUIREMENTS

• Willing to travel up to 100% of the time for business purposes
• Flexibility to travel within assigned serviced areas

Pay: $45.00 - $60.00 per hour *Pay per visit
