Job summary

Evolve Home Health is seeking a Registered Nurse (RN) with home health experience to provide in-home care in Kingsburg, CA. The role involves admissions, assessments, and care planning at patients' homes, with travel across service areas.

Expect travel up to 100% and pay per visit on an hourly basis. Requires CA RN license, 2+ years of home health, BLS/ACLS, and reliable transportation. Schedule typically Monday–Friday with on-call and weekend as needed.

Qualifications

• CA RN license required.
• Minimum 2 years of clinical home health experience.
• Valid Driver's License, reliable transportation, proof of insurance.
• BLS/CPR Certification and ACLS preferred.
• Physical Exam, TB Test, Chest X-ray completed.

Responsibilities

• Provide professional nursing care at patients' homes, perform admissions and ongoing assessments.
• Create and update plan of care based on patient needs.
• Submit nurses notes timely to comply with HIPAA and record-keeping.
• Administer medications and assist with pain management.
• Inspect and care for wounds, monitor symptoms and vital signs.
• Document health data and update care plans as needed.
• Instruct patients and families on proper home care and safety.

Our agencyEvolve Home Healthis located in Agoura Hills, CA,we provide services throughout Los Angeles County and extending into the Mid Central Valley. (Fresno, Clovis, Madera, Coalinga, Mendota, Merced, Tulare, Visalia, San Diego area, and Los Angeles County).

CURRENLY LOOKING FOR AN RN FOR KINGSBURG, CA

ESSENTIAL FUNCTIONS

• Provide professional nursing care, go to the patients home to do admission, and ongoing assessments to patients
• Initiate, develop and update individual appropriate plan of care according to an Individual patients needs
• Submit all nurses notes on a timely manner into the system to be in compliance with HIPPA regulations, maintain accurate records
• Administer medications and assist with pain management
• Clean and dress wounds
• Document symptoms and vital signs
• Monitor patient health and update care plan accordingly
• Instruct patients and their families on proper home care
• Provide suggestions to improve safety at home
• Detect early symptoms that could lead to a hospital visit
• Supervise home health aides
• Exceptional communication with patients, staff, physicians, social workers or other health advisors
• Provide encouragement and support

SKILLS

• Proficiency in using electronic health record (EHR) systems (e.g., DataSoftLogic)
• Familiarity with durable medical equipment (DME) and its use in patient care
• Proficiency in OASIS nursing documentation to ensure completeness, timeliness, and adherence to agency policies and Medicare guidelines. (training provided if needed)
