Field HSS Clinical Coordinator Registered Nurse Level 1 - San Marcos, TX - 2283950

Join us as a Field HSS Clinical Coordinator Registered Nurse Level 1 at Primary Care Plus in San Marcos, TX.

Position Overview

This role offers flexibility for residents within the San Marcos area, with telecommuting options and travel up to 50 miles from the home office as needed. It is a field-based position requiring travel to visit Medicaid members in various settings.

Primary Responsibilities
  • Assess, plan, and implement individualized care strategies focused on the most appropriate, least restrictive level of care.
  • Identify and initiate referrals for social services including financial, psychosocial, and community support programs.
  • Manage the care plan across the continuum as a single point of contact, ensuring communication among stakeholders for coordinated care.
  • Advocate for patients and families to support their needs and choices within the healthcare team.

Qualifications
Required
  • Current, unrestricted RN license in Texas.
  • At least 2 years of experience in community health or related healthcare roles.
  • Intermediate proficiency with Microsoft Outlook, Teams, and web applications.
  • Reliable high-speed internet and a designated quiet workspace at home.
  • Valid US driver’s license and access to reliable transportation for regional travel.
Preferred
  • Experience with long-term care, Medicaid, or Medicare.
  • Knowledge of federal and state home and community-based settings regulations.
  • Reside within a reasonable distance of assigned duties.
