LiveWell Partners’ John Paul VNA Hospice network agency in Michigan is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate and deliver skilled hospice care in patients’ homes.

You will develop and monitor the Plan of Care with the physician and interdisciplinary team, educate families, lead weekly care conferences, ensure regulatory compliance, and maintain EMR documentation to support high-quality patient outcomes.

Qualifications

• Minimum 2 years nursing experience in hospice, palliative, acute, or post-acute care.
• Active state RN licensure.
• Knowledge of hospice regulations and end-of-life best practices.
• CPR certification.
• Valid driver’s license and auto insurance.

Responsabilités

• Assume clinical management of assigned hospice patients from admission to discharge.
• Develop and maintain an individualized Plan of Care with the physician and team.
• Provide direct nursing care including symptom management and crisis intervention.
• Educate patients and families about disease, medications, and hospice services.
• Coordinate and lead weekly care team meetings and interdisciplinary conferences.
• Communicate with physicians, social workers, chaplains, aides, and others for continuity of care.
• Monitor payer requirements and ensure eligibility for hospice services.
• Document care and interactions in the EMR system.
• Supervise and support LPNs and aides; conduct supervisory visits.
• Provide bereavement guidance and psychosocial support to families.
• Participate in 24/7 on-call coverage as assigned.
