JOB DESCRIPTION
Job Summary
The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and receptive, including homes, nursing facilities, and "pop-up" clinics.
The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations. Some programs may focus on specific groups such as adults, geriatrics, pediatrics, or women’s health.
Responsibilities include performing comprehensive medical assessments, ordering tests and procedures, formulating treatment plans, consulting with specialists, and maintaining proper documentation.
Job Duties
- Provide general medical care and coordinate care for various patient groups, including adults, women’s health, pediatrics, and geriatrics.
- Conduct comprehensive evaluations, including history and physical exams, to identify care gaps and perform preventative assessments.
- Address both chronic and acute primary care issues, assessing medical urgency.
- Establish and document medical diagnoses.
- Seek specialist consultations as needed.
- Order and perform diagnostic tests judiciously, working within resource limitations.
- Recognize when patient needs exceed scope of practice, and seek physician oversight when necessary.
- Create and implement medical care plans.
- Schedule patient visits appropriately.
- Coordinate post-discharge care to reduce hospital readmissions and ER visits.
- Perform face-to-face visits in various settings, including home, nursing facilities, and public locations, and via alternative modalities as needed.
- Order targeted laboratory tests for specific populations.
- Participate in on-call coverage for urgent lab and pharmacy inquiries.
- Engage in community-based "Pop Up Clinics" to build community relationships and address healthcare gaps.
- Travel up to 120 miles daily within the assigned region, with potential overnight stays for special projects.
- Obtain and maintain cross-state licensure as required.
- Collaborate with colleagues to develop best practices and participate in regional meetings.
- Prescribe medications and perform procedures as appropriate.
- Document care accurately and timely in electronic medical records.
- Occasionally walk stairs while carrying equipment up to 50 lbs.
Job Qualifications
Required Education
Master's degree in family health from an accredited nursing program.
Required Experience, Skills & Abilities
Proficiency with computer applications such as Word, Excel, and Electronic Medical Records.
Required Licenses, Certifications, and Associations
- Active, unrestricted national certification from recognized organizations (e.g., American Academy of Nurse Practitioners, ANCC).
- Current state-issued license as a Family Nurse Practitioner.
- Basic Life Support (BLS) certification for healthcare professionals.
- Unrestricted driver’s license.
Preferred Education
Additional relevant qualifications or certifications.
Preferred Experience
- 3-5 years as a Registered Nurse or Nurse Practitioner, especially in home, community, or public health.
- Experience managing chronic conditions in home health settings.
- Experience serving underserved populations facing socioeconomic barriers.
- Fluency in additional languages is advantageous.
- Skills in immunization and point-of-care testing.
