JOB DESCRIPTION

Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and receptive, including homes, nursing facilities, and pop-up clinics.

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations. Some programs may target specific groups such as adults, geriatrics, pediatrics, or women’s health.

Responsibilities include performing comprehensive assessments, ordering tests and procedures, formulating treatment plans, consulting with specialists, and documenting all activities appropriately.

Job Duties

  1. Provide general medical care and care coordination for various patient groups, including adults, women’s health, pediatrics, and geriatrics.
  2. Conduct comprehensive evaluations, including history and physical exams, to identify gaps in care and perform preventive assessments.
  3. Address both chronic and acute primary care issues, assessing medical urgency.
  4. Establish and document accurate medical diagnoses.
  5. Seek specialty consultations as needed.
  6. Order and interpret diagnostic tests judiciously, working within resource limitations.
  7. Recognize when patient needs exceed scope of practice and when physician oversight is necessary.
  8. Create and implement medical care plans.
  9. Schedule patient visits appropriately.
  10. Coordinate post-discharge care to reduce hospital readmissions and ER visits.
  11. Perform face-to-face visits in various settings, including homes and clinics, and via alternative modalities when appropriate.
  12. Order targeted laboratory tests for specific populations.
  13. Participate in on-call coverage for urgent lab and pharmacy inquiries, developing appropriate care plans.
  14. Engage in community-based pop-up clinics to build relationships and address healthcare gaps.
  15. Drive up to 120 miles daily within the assigned region, with potential for longer distances and overnight stays for special projects.
  16. Obtain and maintain licensure in multiple states based on business needs.
  17. Collaborate with colleagues to develop best practices and participate in regional meetings.
  18. Prescribe medications and perform procedures as appropriate.
  19. Document all clinical activities timely in electronic medical records.
  20. Occasionally walk stairs carrying equipment up to 50 lbs.

JOB QUALIFICATIONS

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience, Knowledge, Skills & Abilities: Proficiency in computer skills, including Word, Excel, and EMR systems.

Required Licenses, Certifications, and Associations:

  • Active and unrestricted national certification from AANP or ANCC.
  • Current state license as a Family Nurse Practitioner.
  • Current BLS certification for Healthcare Professionals.
  • Valid driver’s license.

Preferred Education and Experience:

  • 3-5 years of experience as a Registered Nurse or Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in home health settings.
  • Experience working with underserved populations facing socioeconomic barriers.
  • Fluency in additional languages is a plus.
  • Skills in immunization and point-of-care testing.
