JOB DESCRIPTION
Job Summary
The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and receptive, including homes, nursing facilities, and pop-up clinics.
The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations. Some programs may target specific groups such as adults, geriatrics, pediatrics, or women’s health.
Responsibilities include performing comprehensive assessments, ordering tests and procedures, formulating treatment plans, consulting with specialists, and documenting all activities appropriately.

JOB QUALIFICATIONS
Required Education: Master's degree in family health from an accredited nursing program.
Required Experience, Knowledge, Skills & Abilities: Proficiency in computer skills, including Word, Excel, and EMR systems.
