Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and receptive, including homes, nursing facilities, and “pop-up” clinics.

Job Description

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations, including adults, geriatrics, pediatrics, and women’s health. Responsibilities include performing comprehensive assessments, ordering tests, formulating treatment plans, consulting specialists, and documenting care.

Job Duties
  • Provide general medical care and care coordination across patient groups.
  • Perform evaluations, including history and physical exams, for gaps in care and preventative assessments.
  • Address chronic and acute complaints, determining medical urgency.
  • Establish diagnoses and seek specialty consultations as needed.
  • Order and interpret diagnostic tests judiciously.
  • Recognize when patient needs exceed scope and require physician oversight.
  • Create and implement medical care plans.
  • Schedule patient visits and coordinate post-discharge care to reduce readmissions.
  • Perform face-to-face visits in various settings, including home and facilities, and via alternate modalities as needed.
  • Order targeted laboratory tests for specific populations.
  • Participate in community-based “Pop Up Clinics” to build community relationships.
  • Drive up to 120 miles daily within the region, with potential overnight stays for special projects.
  • Obtain and maintain cross-state licenses as required.
  • Collaborate with colleagues to develop best practices.
  • Participate in regional meetings and activities.
  • Prescribe medications and perform procedures as appropriate.
  • Document timely and accurately in electronic medical records.
  • Occasionally walk stairs carrying up to 50 lbs. of equipment.
Job Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and Electronic Medical Records.

Required Licenses/Certifications:

  • Active and unrestricted national certification (e.g., AANP, ANCC).
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support certification.
  • Unrestricted driver’s license.
Preferred Qualifications
  • 3-5 years of experience as RN or Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in underserved populations.
  • Fluency in additional languages; immunization and point-of-care testing skills are a plus.
