The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable, including homes, nursing facilities, and "pop-up" clinics.

The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations, possibly focusing on specific groups such as adults, geriatrics, pediatrics, or women’s health. Responsibilities include performing comprehensive assessments, ordering tests, formulating treatment plans, consulting specialists, and documenting care.

Job Duties
  • Provide general medical care and care coordination for various patient groups.
  • Perform evaluations including history and physical exams, addressing care gaps and preventive assessments.
  • Manage both chronic and acute primary care issues, assessing urgency.
  • Establish diagnoses and seek specialty consultations when needed.
  • Order and interpret diagnostic tests judiciously, especially in resource-limited environments.
  • Recognize when patient needs exceed scope and involve physicians accordingly.
  • Create and implement medical care plans.
  • Schedule and conduct patient visits in various settings, including home and facilities.
  • Coordinate post-discharge care to reduce readmissions and ER visits.
  • Perform face-to-face visits and possibly virtual visits based on needs and regulations.
  • Order targeted laboratory tests for specific populations.
  • Participate in on-call coverage for urgent issues.
  • Engage in community-based "Pop Up Clinics" to improve healthcare access.
  • Drive up to 120 miles daily within the region, with potential overnight stays for extended projects.
  • Obtain and maintain cross-state licenses as needed.
  • Collaborate with colleagues to improve practices.
  • Participate in regional meetings.
  • Prescribe medications and perform procedures as appropriate.
  • Document all care timely in electronic medical records.
  • May need to walk stairs with equipment and carry up to 50 lbs.
Job Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and electronic medical records; advanced computer skills.

Required Licenses and Certifications:

  • Active, unrestricted national certification (e.g., AANP, ANCC).
  • Current state license as a Family Nurse Practitioner.
  • Current BLS certification.
  • Unrestricted driver’s license.
Preferred Qualifications

3-5 years of experience as a RN or Nurse Practitioner, preferably in home or community health, managing chronic conditions, and serving underserved populations. Bilingual skills and point-of-care testing experience are a plus.
