Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and receptive, including homes, nursing facilities, and "pop-up" clinics.

Job Description

The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations, including adults, geriatrics, pediatrics, and women’s health. Responsibilities include performing comprehensive assessments, ordering tests, formulating treatment plans, consulting specialists, and documenting care.

Job Duties
  • Provide general medical care and care coordination for various patient groups.
  • Perform evaluations, including history and physical exams, to identify care gaps and conduct preventive assessments.
  • Address both chronic and acute complaints, assessing medical urgency.
  • Establish diagnoses and seek specialist consultations when needed.
  • Order and interpret diagnostic tests judiciously, working within resource limitations.
  • Recognize when patient needs exceed scope and require physician oversight.
  • Create and implement medical care plans.
  • Schedule appointments and coordinate post-discharge care to reduce readmissions and ER visits.
  • Conduct face-to-face visits in various settings, including home and clinics, and via alternate modalities as needed.
  • Order laboratory tests targeting specific populations.
  • Participate in community clinics and drive within the region, possibly exceeding 120 miles daily, including overnight stays for special projects.
  • Obtain and maintain cross-state licenses as required.
  • Collaborate with colleagues to develop best practices and participate in regional meetings.
  • Prescribe medications and perform procedures as appropriate.
  • Document all care timely in electronic medical records.
  • May need to walk stairs with equipment and carry up to 50 lbs.
Job Qualifications

Required Education: Master’s degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and electronic medical records.

Required Licenses & Certifications:

  • Active, unrestricted national certification (e.g., AANP, ANCC).
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support certification.
  • Unrestricted driver’s license.

Preferred Qualifications:

  • 3-5 years of RN or NP experience, especially in home or community health.
  • Experience managing chronic conditions and working with underserved populations.
  • Additional language skills and immunization/testing skills are a plus.
