Job Summary

Job Description

The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations. Some programs may focus on specific groups such as adults, geriatrics, pediatrics, or women’s health. Responsibilities include performing comprehensive assessments, ordering tests, formulating treatment plans, consulting specialists, and documenting care.

Job Duties
  • Provide general medical care and care coordination for various patient groups.
  • Perform evaluations including history and physical exams to identify care gaps and preventative needs.
  • Manage chronic and acute conditions, assessing medical urgency.
  • Establish diagnoses and seek specialist consultations as needed.
  • Order and interpret diagnostic tests judiciously.
  • Recognize when patient needs exceed scope of practice and involve physicians accordingly.
  • Create and implement medical care plans.
  • Schedule patient visits and coordinate post-discharge care to reduce readmissions.
  • Conduct face-to-face visits in various settings, including home and community locations.
  • Utilize alternate modalities for visits based on needs and regulations.
  • Order targeted laboratory tests for specific populations.
  • Participate in community clinics and drive within the region, possibly exceeding 120 miles daily or staying overnight for projects.
  • Obtain and maintain cross-state licenses as required.
  • Collaborate with colleagues and participate in regional meetings.
  • Prescribe medications and perform procedures as appropriate.
  • Ensure timely documentation in electronic medical records.
  • Perform physical tasks, including walking flights of stairs and carrying equipment.
Job Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and electronic medical records.

Required Licenses/Certifications: Active national certification (e.g., ANCC, AANP), current state license, BLS certification, and valid driver’s license.

Preferred Qualifications
  • 3-5 years of experience as a nurse or nurse practitioner, especially in home or community health.
  • Experience managing chronic conditions and working with underserved populations.
  • Additional language skills and immunization/testing skills are a plus.
