JOB DESCRIPTION
Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide care in environments where patients feel most comfortable, including home, nursing facilities, and "pop up" clinics.

The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations. Some programs may target specific groups, such as adults, geriatrics, pediatrics, or women's health.

Responsibilities include performing comprehensive assessments, ordering diagnostic tests, formulating treatment plans, consulting specialists, and documenting care as required.

Job Duties
  1. Provide general medical care and coordination for various patient groups, including adults, women’s health, pediatrics, and geriatrics.
  2. Perform comprehensive evaluations, including history and physical exams, to identify care gaps and preventive needs.
  3. Address both chronic and acute primary care issues, assessing medical urgency.
  4. Establish and document accurate medical diagnoses.
  5. Seek specialist consultations when appropriate.
  6. Order and interpret diagnostic tests judiciously, working within resource limitations.
  7. Recognize when patient needs exceed scope and require physician oversight.
  8. Create and implement medical care plans.
  9. Schedule patient visits as needed.
  10. Coordinate post-discharge care to reduce readmissions and ER visits.
  11. Conduct face-to-face visits in various settings, including home and facilities, and via alternative modalities as needed.
  12. Order targeted laboratory tests for specific populations.
  13. Participate in on-call coverage for urgent issues.
  14. Engage in community "Pop Up Clinics" to build relationships and address healthcare gaps.
  15. Travel up to 120 miles daily within the region, with possible overnight stays for special projects.
  16. Obtain and maintain licenses in multiple states as required.
  17. Collaborate with colleagues to optimize work practices.
  18. Participate in regional meetings.
  19. Prescribe medications and perform procedures as appropriate.
  20. Maintain timely and accurate documentation in electronic medical records.
  21. Occasionally perform physical tasks, including walking stairs with equipment.
Job Qualifications
Required Education

Master's degree in family health from an accredited nursing program.

Required Experience, Knowledge, Skills & Abilities

Proficiency with Word, Excel, and Electronic Medical Records.

Required Licenses, Certifications, & Associations
  • Active, unrestricted national certification from AANP or ANCC.
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support certification for Healthcare Professionals.
  • Unrestricted driver’s license.
Preferred Education

Additional degrees or certifications are a plus.

Preferred Experience
  • 3-5 years as RN or Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in home health.
  • Experience with underserved populations and socioeconomic barriers.
  • Fluency in additional languages is advantageous.
  • Skills in immunization and point-of-care testing.
