Job Summary
The Nurse Practitioners focus on screening and preventive primary care in various settings, providing care to all patient levels, including specific populations such as adults, geriatrics, pediatrics, and women’s health. They perform comprehensive assessments, order tests, formulate treatment plans, consult specialists, and document care appropriately.

Job Duties
  • Provide general medical care and care coordination for diverse patient groups.
  • Perform evaluations, including history and physical exams, to identify gaps in care and conduct preventive assessments.
  • Address chronic and acute primary care issues, assessing medical urgency.
  • Establish diagnoses, seek specialist consultations, and order diagnostic tests judiciously.
  • Recognize when patient needs exceed scope and involve physicians as needed.
  • Create and implement medical care plans.
  • Schedule patient visits and coordinate post-discharge care to reduce readmissions and ER visits.
  • Conduct face-to-face visits in various settings, including home and clinics, and via alternate modalities as needed.
  • Order laboratory tests targeting specific populations.
  • Participate in community clinics and outreach activities.
  • Drive up to 120 miles daily within the region, with potential overnight stays for special projects.
  • Obtain and maintain cross-state licenses as required.
  • Collaborate with colleagues to develop best practices and participate in regional meetings.
  • Prescribe medications and perform procedures as appropriate.
  • Document in electronic medical records timely and accurately.
  • Occasionally walk flights of stairs while carrying equipment.
Job Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and EMR systems.

Required Licenses & Certifications:

  • Active national certification from AANP or ANCC.
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support certification.
  • Valid driver’s license.

Preferred Education & Experience:

  • 3-5 years in nursing or as a Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in underserved populations.
  • Multilingual abilities and skills in immunization/testing are a plus.
