Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable, including home, nursing facilities, and “pop-up” clinics.

Job Description

The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations, including adults, geriatrics, pediatrics, and women’s health. Responsibilities include performing assessments, ordering tests, formulating treatment plans, consulting specialists, and documenting care.

Job Duties

• Provide general medical care and care coordination for various patient groups.
• Perform evaluations, including history and physical exams, and address care gaps and preventive assessments.
• Manage both chronic and acute primary care issues, assessing urgency.
• Establish diagnoses, seek specialist consultations, and order diagnostic tests judiciously.
• Develop and implement medical care plans, schedule appointments, and coordinate post-discharge care.
• Conduct face-to-face visits in homes, nursing facilities, or public locations; may include telehealth modalities.
• Drive within the assigned region, potentially exceeding 120 miles/day, and obtain necessary cross-state licenses.
• Collaborate with colleagues, participate in regional meetings, prescribe medications, and document in electronic medical records.
• Perform physical tasks, including walking stairs with equipment as needed.

Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and EMR systems; active and unrestricted Nurse Practitioner certification; current state license; BLS certification; valid driver’s license.

Preferred Qualifications: 3-5 years of experience, especially in home or community health, with underserved populations; additional language skills; immunization and point-of-care testing skills.
