Job summary

Lightways Hospice in Joliet, IL is seeking a Field CNA to join our team. The role supports patients in home, facility, or other setting under RN supervision, with a schedule Monday–Friday 8:00 AM–4:30 PM.

Responsibilities include providing compassionate care, assisting with daily activities, monitoring patient condition, and ensuring safety and equipment use. Travel within Joliet and surrounding areas is required.

Qualifications

• High school diploma or GED.
• Meets the training requirements of Illinois.
• Current CNA certification.
• Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.

Responsibilities

• Provide compassionate care, emotional support, and companionship to patients and families.
• Assist with personal care, mobility, and daily living activities.
• Monitor, document, and report patient condition and changes to the RN.
• Ensure patient safety and proper use of equipment.
• Maintain a clean, safe, and comfortable home environment.
• Follow care plans, policies, and participate in training and team activities.
• Position requires outside travel in Joliet and surrounding area
