As part of a support coordination team managing members enrolled in the Employment and Community First CHOICES (ECF CHOICES) program, the ECF CHOICES Support Coordinator will serve as the primary support for members with intellectual or developmental disabilities. Responsibilities include supporting members' medical, behavioral, and socioeconomic needs to promote appropriate service utilization and improved quality of care.

If you reside in Tennessee, you will have the flexibility to work remotely* as you tackle challenging issues.

Primary Responsibilities:
  1. Engage members via face-to-face or telephonic assessments to evaluate medical, behavioral, functional, cultural, and socioeconomic (SDoH) needs.
  2. Develop and implement person-centered care plans addressing management of chronic health conditions, health promotion, social determinants, medication management, and safety, aligned with evidence-based guidelines.
  3. Create individualized care plans with goals, opportunities, and interventions suited to the member’s readiness to change, supporting optimal health and quality of life.
  4. Collaborate with internal care teams, providers, and community resources to execute care plans.
  5. Provide referrals and linkages to internal and community-based providers as appropriate and accepted by the member.

Required Qualifications:
  • One of the following:
    • 4-year degree in health, human services, or behavioral health fields
    • Unrestricted independent licensure as a Social Worker
    • LPN with 5+ years of direct experience
    • 3+ years of experience with DD or IDD Waivers
  • 2+ years serving individuals with disabilities
  • 1+ years working with community providers addressing SDoH
  • Proficiency in MS Office and recent experience with Adobe and MS Teams
  • Driver's license, reliable transportation, and ability to travel up to 75% within East TN
Preferred Qualifications:
  • Community case management experience for complex needs
  • Training in Motivational Interviewing, Trauma-Informed Care, etc.
  • Experience in team-based care and serving individuals with co-occurring disorders
  • Background in Managed Care
Physical Requirements:
  • Ability to transition between office and field locations
  • Navigate various terrains and transport equipment
  • Remain stationary for computer work

*Remote work requires adherence to UnitedHealth Group's Telecommuter Policy.
