As part of a support coordination team managing members enrolled in the Employment and Community First CHOICES (ECF CHOICES) program, the ECF CHOICES Support Coordinator will serve as the primary support for members with intellectual or developmental disabilities. Responsibilities include supporting medical, behavioral, and socioeconomic needs to promote appropriate service utilization and improve quality of care.

This is a home-based position with field responsibilities approximately 50-75% of the time, traveling within Sumner, Wilson, Cannon, and Davidson counties. Remote work flexibility is available for Tennessee residents.

Primary Responsibilities
  • Engage members face-to-face and/or telephonically for comprehensive needs assessments, including medical, behavioral, functional, cultural, and socioeconomic needs.
  • Develop and implement person-centered care plans addressing chronic conditions, health promotion, social determinants, medication management, and safety, aligned with evidence-based guidelines.
  • Create individualized care plans with goals and interventions tailored to the member’s readiness to change to optimize health and quality of life.
  • Partner with internal care teams, providers, and community resources to implement care plans.
  • Provide referrals and linkages to internal and community providers as appropriate and accepted by the member.

Required Qualifications
  • Degree in a health, human service, or behavioral health field (e.g., Social Work, Nursing, Psychology, Counseling) plus 3+ years of experience OR current, unrestricted Social Work license OR LPN with 5+ years of experience.
  • 2+ years serving individuals with disabilities.
  • 1+ year working with local home/community providers and addressing social determinants of health.
  • Proficiency in MS Office.
  • Reliable transportation and ability to travel within specified counties.
Preferred Qualifications
  • Community case management experience for individuals with complex needs.
  • Training in motivational interviewing, trauma-informed care, person-centered care.
  • Experience with team-based care, co-occurring disorders, managed care background.

The hourly pay ranges from $28.61 to $56.06, based on experience and other factors. Benefits and incentives are offered, consistent with company policies.
