Qualifications

• 1+ years of experience in field-based roles.
• Excellent written and verbal communication skills.
• Demonstrates empathy and emotional intelligence.
• Ability to travel daily and have own reliable transportation.
• Proactive, self-starter with the ability to work independently.

Responsibilities

• Manage an assigned patient's caseload.
• Adhere to monthly service and compliance standards.
• Direct point-of-contact for enrolled member questions.
• Experience with data input, case review, and building rapport with patients and caregivers.
• Collaborate with Care Coordinators to schedule home visits.
• Conduct home visits and document to meet compliance.
• Review home visit results with Care Coordinators and plan follow-up.
