NIVA Health is seeking a compassionate Mobile Wound Care Nurse (LPN/LVN) to join our field-based team. You will provide advanced wound care therapies directly in patients' homes, helping them heal and regain independence.

This full-time role involves traveling to local patient locations, documenting in the EMR, and collaborating with an interdisciplinary team. An active LPN/LVN license and driver's license are required.

Qualifications

• Active LPN/LVN license in the state of practice.
• Willing to travel locally to patient homes.
• Experience in wound care preferred.

Responsibilities

• Provide advanced wound care therapies in patient homes.
• Travel locally to patients’ homes following safety protocols.
• Collaborate with an interdisciplinary care team.
