Job summary

A Plus Hire Right Search Group is seeking a Nurse Practitioner or Physician Assistant for a full-time wound-care role in Richmond, VA. The clinician will evaluate and treat patients in homes and post-acute settings, with extensive training and ongoing education provided to develop wound medicine expertise.

Responsibilities include patient assessments, care plan development, independent clinical decisions, and thorough documentation.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in good standing.
• Ability to practice independently.
• Strong assessment, documentation, and communication skills.
• Comfortable caring for medically complex adult patients.
• Wound care experience not required but preferred.

Responsibilities

• Evaluate and treat patients with chronic and complex wounds in home or post-acute settings.
• Conduct comprehensive patient assessments and develop individualized plans of care.
• Write clinical orders and make independent clinical decisions.
• Communicate effectively with patients, families, nurses, and other healthcare professionals.
• Maintain accurate and timely clinical documentation.
• Travel locally throughout the workday to patient homes and facilities.
