Qualifications

• Requires MS/MA in social work, counseling, or related behavioral health field or a nursing degree with 3+ years of clinical experience.
• Active, current license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist as applicable by state law.

Responsibilities

• Collaborate with families, foster parents, and service teams to create individualized treatment plans.
• Conduct assessments to identify member needs.
• Develop comprehensive care plans addressing identified objectives and goals.
• Support access to quality, cost-effective care and adjust plans as needed.
• Coordinate with internal and external resources for whole-person care.
• Address social determinants of health within care planning and collaborate with state agencies.
• Maintain knowledge of system-of-care philosophy and community supports.
• Build partnerships with populations and families, addressing cultural/linguistic needs.
• Evaluate health needs and identify applicable services and resources with families.
• Provide patient education, medication reconciliation, and arrange follow-up care.
• Facilitate health rounds with clinicians to ensure integrated care.
• Lead or participate in specialty projects and process improvements.
• Train and mentor staff; perform audits and ensure compliance.
• Serve as a member of the leadership team.
