Job summary

Matter Health is seeking a Patient Advocate & Liaison to connect with older adults in public housing communities across Tennessee. The PAL educates patients, supports enrollment, and coordinates follow-up to keep patients engaged with our care model.

The role partners with NP-led teams to remove barriers to care, conduct outreach, and document activities in Salesforce and Athena. Field-based with community-based impact.

Qualifications

• 1–2 years in community outreach, patient advocacy, healthcare, social services, or customer-facing roles.
• Passion for serving older adults and underserved communities.
• Excellent communication and relationship-building skills.
• Able to work independently in a field-based role.
• Strong organizational and time-management abilities.
• Collaborates well with clinical and operations teams.
• Valid driver's license and reliable transportation.
• Experience with Medicare populations, Salesforce, or Athena is a plus.
• Bilingual Spanish proficiency is a plus.

Responsibilities

• Build trusted relationships with older adults in assigned communities.
• Serve as the primary Matter Health representative in the community.
• Educate prospective patients about Matter Health and enrollment options.
• Support outreach, enrollment, scheduling, and ongoing engagement.
• Conduct in-person outreach, calls, and community events.
• Collaborate with APPs, social workers, care navigators, and operations teams.
• Identify barriers to care and connect patients with resources.
• Maintain accurate documentation in Salesforce and Athena.
• Help improve patient retention and access to services.
