Job summary

The Center for Health Care Services in San Antonio seeks a behavioral health clinician to provide direct and indirect services to adults, delivering trauma-informed, person-centered care across home, clinic, and community settings.

You'll work within a multidisciplinary team, develop treatment plans, conduct assessments, coordinate intake, document in EMR, and participate in training and quality improvement activities, including discharge planning and care coordination.

Qualifications

• Master’s degree in social work, psychology, counseling, or related behavioral health field.
• QMHP-CS within six weeks of hire or transfer.
• Licensed Professional Counselor Associate (LPC-A) in Texas for JIAA programs.

Responsibilities

• Develops new/temporary programs in response to community needs.
• Conducts assessments and develops treatment plans for consumers.
• Coordinates intake to Adult Behavioral Health in designated programs.
• Documents clinical interactions in EMR and other platforms.
• Provides case management, care coordination, discharge planning, referrals, and transition support.
• Trains, mentors and coaches new employees.
• Performs other related duties as required.
