Job summary

Abernathy Home Care is seeking a compassionate RN or LPN to join our field-based nursing team in Massachusetts. You will travel to client homes to perform assessments and supervisory visits, supporting seniors and adults to live safely and independently at home.

This full-time role offers a Monday–Friday schedule with no weekends, mileage reimbursement, and a competitive salary range of $90,000–$105,000 per year based on experience.

Qualifications

Active RN or LPN license in Massachusetts
Home care experience preferred, but not required
New graduates are encouraged to apply
ASAP/community-based care experience strongly preferred
Strong assessment, communication, and documentation skills

Responsibilities

Conduct in-home client assessments and reassessments
Complete supervisory visits for HHA/PCA caregivers
Monitor client needs, conditions, and changes in care
Ensure services are provided safely and in accordance with applicable program requirements
Communicate and collaborate with ASAP case managers, caregivers, clients, and families
Maintain accurate and timely clinical documentation
Communicate changes, concerns, and recommendations to the care team
Help support quality, continuity, and compliance across client services
