Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a Field Based RN II to assess members, develop and coordinate care plans, and partner with care teams to ensure appropriate services across settings.

Qualified candidates will hold an active NJ RN license, have at least two years of clinical experience, and ideally possess a Bachelor's degree. This field-based role requires daily local travel and a valid New Jersey driver’s license, with a strong commitment to member-centric care.

Qualifications

• Education/Experience: High School Diploma/GED; Bachelor degree preferred or relevant experience in lieu of degree.
• Requires a minimum of two (2) years clinical experience.

Responsibilities

• Assesses member's clinical need against guidelines to ensure medical appropriateness.
• Evaluates necessity and efficiency of services.
• Develops and coordinates a member-centered plan of care.
• Coordinates with patient, family, physician, hospital and external customers.
• Monitors care and outcomes for appropriateness and effectiveness.
• Advocates for members across sites to coordinate resources.
