Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based RN with 2+ years of clinical experience to assess member needs and develop personalized plans of care. You will coordinate with families, physicians, and facilities to ensure medically appropriate, cost-effective care across the continuum.

Applicants must hold an active NJ RN license and be prepared for daily local travel in the New Jersey area. The role offers comprehensive benefits and a mission-driven work environment.

Qualifications

• High School Diploma or GED required.
• Bachelor degree preferred or relevant experience in lieu of degree.
• Requires a minimum of two (2) years clinical experience.
• Active Unrestricted NJ RN License required.

Responsibilities

• Assesses member's clinical need against guidelines to ensure services are medically appropriate.
• Evaluates necessity, appropriateness and efficiency of services provided.
• Develops, coordinates and assists in implementation of member's plan of care.
• Coordinates with patient, family, physician, hospital and external customers regarding care.
• Advocates for member/family across sites to coordinate resource utilization and evaluation of services.
