Adaptive Home Health, LLC is seeking a full-time RN Case Manager to deliver skilled home health care across Texas. In this role, you will own patient assessments, provide wound care, and manage infusion therapy, while also advocating for patients and ensuring continuity of care.

The ideal candidate will have a current Texas RN license, 3+ years of clinical nursing experience, and strong communication skills.

Qualifications

• Must have a current Texas RN license.
• 3+ years of clinical nursing experience required.
• Comfort with wound care and infusions is essential.
• CPR certification required (BLS).
• Current Texas driver’s license needed.
• Strong time management and organization skills.

Responsibilities

• Own patient assessments and skilled visit documentation.
• Deliver high-acuity home health care.
