Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) - Case Manager to deliver skilled home health care across Texas. You will own patient assessments, document visits, and coordinate care with physicians and the care team.

You will work in a field-based, autonomous role with an emphasis on wound care and infusion therapy, and you’ll be paid per productivity point with an expectation of 30 points per week. Schedule is Monday–Friday with some weekends and on-call support.

Qualifications

• Texas RN license active
• 3+ years clinical nursing experience
• Comfort with wound care and infusions
• CPR (BLS)
• Previous home health experience
• Current Texas driver’s license and field-based work
• Strong time management and organization in a high-autonomy environment
• Clear, professional communication with patients, families, and providers
• Experience coordinating care for medically complex patients (Nice-to-have)
• ER or equivalent nursing experience (Nice-to-have)

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians, care team, patients, and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination and follow-ups.
• Coordinate changes in condition, track orders, and coordinate services to execute the plan of care.
• Administer medications and provide patient and caregiver teaching.
• Collaborate with internal teams to ensure reliable and safe care delivery.
• Provide supervision to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality and outcomes.
