Qualifications

Responsibilities

• Lead care management with PCP collaboration to reduce admissions.
• Own care coordination and ensure alignment across the care team.
• Manage transitions of care with timely assessments and medication reconciliation.
• Document visits in the EHR and participate in in-person planning meetings.
• Educate patients and families, advocate for needs, and ensure care plan progression.
