Job summary

Molina Healthcare, Inc. is seeking an RN cared-manager to support LTSS care coordination and cross-disciplinary collaboration.

The role focuses on assessing member needs, developing and monitoring care plans, and facilitating access to waiver services across diverse settings. The successful candidate will navigate complex care coordination, document interventions, and work with a team to improve outcomes while traveling to member sites as required.

Qualifications

• Active RN license in state of practice.
• At least 2 years in health care with care management or LTSS exposure.
• Ability to operate proactively and handle multiple priorities.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Manage waiver enrollment/disenrollment processes and develop care plans.
• Coordinate with interdisciplinary team to ensure integrated member care.
