Job summary

Humana in Eau Claire, WI is hiring an RN Care Manager to support members enrolled in the Wisconsin Family Care Program. This full-time, Monday–Friday role offers a daytime schedule (8:00 am–4:30 pm CST) with no night or weekend shifts and up to 40% travel.

You will assess members, develop MCPs, conduct health assessments, coordinate with providers, educate on disease processes and lifestyle changes, document promptly, and support newly assigned RNs with follow-up.

Qualifications

• Education: Associate degree in nursing required
• Experience: 3 years Care Management experience preferred
• Licenses & Certifications: Licensed Registered Nurse in the state of Wisconsin (in good standing)

Responsibilities

• Assess members to develop a comprehensive Member Care Plan (MCP)
• Conduct regular health assessments
• Coordinate with acute and primary care providers
• Educate members on disease processes, nutrition, and lifestyle modifications
• Document in member files within required DHS contract timeframes
• Participate in member monthly phone contacts and travel for in-person visits
• Complete record reviews for reassigned members
• Provide support regarding record review and remediation
• Ensure newly assigned RNs are informed of any missing or needed follow-up
• Accept additional complex member assignments
