Job summary

One Love Agency is seeking compassionate Case Managers (CM) and Community-Based Rehabilitative Specialists (CBRS) to help adults reintegrate after incarceration. Work in Boise and surrounding communities, primarily in the field, with some office access.

Flexible schedule and opportunities for leadership and advancement are highlighted. You will assess needs, develop plans, and connect clients with housing, employment, health services, transportation, and community resources to support stability

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or a related field preferred.
• Experience working with mental health, substance use, justice-involved populations, or community-based services preferred.
• Strong communication, organization, and problem-solving skills.

Responsibilities

• Provide Case Management and/or CBRS services to adults with mental health, behavioral health, and reintegration needs.
• Complete assessments and assist clients in identifying strengths, barriers, goals, and support needs.
• Develop individualized service plans in collaboration with clients and their support systems.
• Assist clients with accessing housing, employment opportunities, educational resources, healthcare, transportation, and community services.
• Teach and reinforce independent living skills, coping skills, problem-solving skills, communication skills, and community integration strategies.
• Support clients in reducing barriers that contribute to instability and recidivism.
• Advocate for clients and assist them in navigating complex systems and community resources.
• Collaborate with probation and parole officers, treatment providers, employers, housing agencies, and other community partners.
• Monitor client progress and adjust interventions as needed to support successful outcomes.
• Maintain accurate, timely, and compliant documentation in accordance with Medicaid and agency standards.
