CFS Corp. is seeking a community outreach professional to connect individuals in crisis with treatment and peer recovery support across three counties. You will build referral networks, represent the agency at health fairs, and coordinate warm handoffs to clinicians and peers.

This role requires evening/weekend availability, strong presentation skills, and deep knowledge of local services to support successful enrollment and ongoing engagement.

Qualifikationen

• Two years of experience in outreach, referral coordination, intake, care coordination, or recovery support.
• Working knowledge of opioid use disorder, MAT, and peer recovery services.
• Strong local knowledge of Knox, Daviess, and Martin Counties.
• Confidence presenting to professionals and connecting with people in difficult moments.
• High school diploma or equivalent; degree in related fields preferred.

Aufgaben

• Build the network by developing referral relationships with hospitals, courts, probation, DCS, schools, EMS, and community providers.
• Represent CFS, Corp. at health fairs, coalition meetings, and partner sites.
• Open the door: gather initial information and coordinate warm handoffs to front desk, clinicians, and peer recovery coaches.
• Keep people connected: reach out to participants who missed appointments and solve barriers like transportation.
• Measure what matters: track referrals from first contact through enrollment and report results to leadership.
