You will be part of the primary care team collaborating with all interdisciplinary team members and supporting multiple providers. The Patient Care Coordinator is a flexible team member who works to support nurse practitioners (NPs) and physician assistants (PAs) as well as our patients. This role involves care coordination activities, administrative tasks, and supporting our patients and providers.

Position Details
This is a full-time position, Monday - Friday, with flexible 8-hour shifts during our normal business hours of 8:00 am - 5:00 pm. Occasional overtime may be required. We offer 2 months of paid training, with hours to be discussed on your first day.

Primary Responsibilities

  • Support interdisciplinary care team with member coordination under RN or NP/PA guidance
  • Data entry and member chart management (new members, post-hospital visits, labs, medications)
  • Obtain medical records following hospitalizations or physician visits
  • Schedule member transportation or appointments

Qualifications

  • High school diploma/GED or equivalent experience
  • Must be 18+ years old
  • 3+ years clinical or medical records experience, with strong medical terminology and EHR knowledge
  • Reliable transportation and ability to travel up to 100%
  • Reside in Berks or Lehigh, PA
  • Flexibility to work full-time, Monday - Friday, during business hours, with occasional overtime

Preferred Qualifications

  • Certified nursing assistant or medical assistant certifications
