Job summary

Adaptive Home Health is seeking an Occupational Therapist to deliver in-home OT services in Detroit and surrounding areas. • Active Michigan OT license in good standing.
• Demonstrated clinical judgment in field settings.
• Strong communication and documentation skills.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Develop and implement plan of care focused on ADLs/IADLs, safety, and independence.
• Recommend adaptive equipment and home modifications as needed.
• Coordinate with nursing and care team when conditions change.
